Please read and print the following information before scheduling your interview with Full
Life also, please print and sign our
Reference Release form and bring it with you to your
interview.


Prior to employment you are required to provide:
•        Driver’s license (if your positions involves driving while at work)
•        Auto insurance card and declaration (policy) page.
•        Proof of legal ability to work in the United States. (
I-9 Documentation)
•        CPR card (classes may be arranged through Full Life.)
•        First aid card (classes may be arranged through Full Life.)
•        Two step TB clearance (information below.)
•        Full Life Orientation Completion

Your employment is also contingent upon:
•        Qualifying traffic and criminal abstract
•        Must be able and willing to pass a Child Welfare Services (CPS) and Adult Protective Services
         (APS) clearance check.
•        References/Background check.
•        Successful interview with the Full Life participant you are going to work with.

TB TESTS
Full Life employees are required to have a 2-Step TB test before they can begin work.  Please print
and bring the
TB Test Provider letter with you when you go to get your test.  (it explains why you
need a two-step, rather than a single step) If you have had one TB test with in the last year and
have a record of it you will only need to obtain one more TB test and provide copies of both these
tests to Full Life.  If you have had a previous positive TB test, please contact Full Life for additional
instructions.

TB tests may be obtained for free from the Hawaii State Public Health Office.
West Hawaii
Public Health is located below the Kona Community Hospital in Kealakekua.
Tests are given on Fridays only between 2:00 PM and 4:00 PM and read the following Monday
between 2:00 PM and 4:00 PM.  You will receive your card at that time.
If the test is a 2-Step you will need to repeat the process the following week. (You will receive your
card at the end of the two-week period).
Bring a valid driver’s license or picture ID.

The Salvation Army also provides TB tests for an $11.00 charge.  
They are located in the Village Professional Building across from the Hawaii Employees Credit Union
on Hualalai.

East Hawaii
TB tests may be obtained for free from the Dept. Of   Health Office.
They are located at 191 Kuawa  St. Turn on Kuawa  Street  from Manono  Street , located on the
left hand side in the Waiakea  Health Center across of the baseball field.  Tests are given on
Mondays only between 1:00 PM and 3:00 PM and read Thursdays between 1:00 PM and 3:00 PM.  
You will receive your card at that time.   
Bring a valid driver’s license or picture ID.

The Bay Clinic in Hilo also provides TB tests  
They are located at 311 Kalanianaole  Avenue, from Banyan Drive - go past Verna’s Drive Inn and
go towards the beach road, it is located on the left side of the Hwy.  Testing is done everyday
except Thursdays also, Fridays if the following Monday is a Holiday.  Walk-ins are from 8:00 AM –
11:00 AM and 1:30PM – 4:00 PM.  Readings are done 2 days later.
The cost is determined on your income and insurance.  The least amount of co pay is $10.00 and
the most is $32.00, they go according to your income and if you have insurance that covers, it
would be free.

CPR and First Aid Certification
Full Life employees are required to have current CPR and First Aid certification and to have this on
file at the Full Life office.

West Hawaii-
The CPR and First Aid classes are held the same week as orientation, in our Full Life office.  Full Life
pays the class dues for all hired employees to attend this class.  Certification may also be obtained
at one of the following depending on availability:

Kona YMCA (Located in the Kailua-Kona, Old Industrial).

Classes are usually scheduled for the first Monday and Wednesday of each month (CPR on one
night and First Aid on the other), but they seem prone to cancellations.

East Hawaii-
You will receive a voucher to attend a class with Barbera Shelton, paid by Full Life.  Certification is
also available at the American Red Cross.
