Join Full Life & Access Support Services

Contact Full Life for a tour and/or to learn more about joining Full Life’s services. Full Life can hire family members as paid Direct Support Professionals supporting their loved ones and/or we can help you identify the right support staff for you and your family member.


If you are not currently enrolled with the Hawai‘i State Department of Health, Developmental Disabilities Division (DOH-DDD), then start on your journey with Full Life by following the steps below to access our services through DOH-DDD.

  • Step 1: Check Your Eligibility

    Before applying, ensure you meet the eligibility criteria:


    • Citizenship and Residency: You must be a U.S. citizen, national, or permanent resident and a resident of Hawaii.
    • Disability Requirements: You need a diagnosis of an intellectual or developmental disability that meets DDD’s criteria.
  • Step 2: Gather Necessary Documentation

    Collect all required information to support your application, including:


    • Proof of Citizenship and Residency: Such as a birth certificate or green card.
    • Medical and Psychological Evaluations: Including any diagnoses and assessments of your disability.
    • School Records and Assessments: If applicable, like your Individualized Education Program (IEP).
    • Legal Documentation: Guardianship documents if you are over eighteen.
  • Step 3: Contact DDD

    Reach out to the DDD office in your area to schedule a meeting with an intake worker:


    • Find your local DDD contact information here 
    • It’s recommended that you attend this meeting personally, even if you have assistance.
  • Step 4: Submit Your Application

    • Initial Meeting: Provide all your gathered documents to your intake worker at this meeting.
    • Additional Information: After the review, you might be asked to provide more information to support your application.
  • Step 5: Wait for Eligibility Determination

    • After submitting all required documents, DDD has thirty (30) working days to make an eligibility determination.
    • You will receive notification of whether you are eligible or not.
  • Step 6: Develop Your Individualized Service Plan (ISP)

    • If eligible, you will be assigned a case manager to develop an ISP that outlines your services, goals, and the supports needed to achieve them.
  • Step 7: Choose Full Life Hawaii

    • Once your ISP is set, you will be presented with a list of approved service providers.
    • Select Full Life Hawaii to start receiving personalized care and support tailored to your needs.
